Crestwood’s programs in Vallejo are based on a commitment to providing mental health consumers a continuum of care that puts them well on the road to recovery. They include  Crestwood Recovery and Rehabilitation Center and Crestwood Manor. As with other Crestwood programs, many of our consumers participate in the innovative Wellness Recovery Action Plan (WRAP) treatment model.

Consumers at Crestwood Recovery and Rehabilitation Center participate in a largely self-initiated recovery process, directed by their individual WRAP. Consumers progress through the structured levels of our program at their own pace, with support every step of the way. We also offer a diverse array of classes ranging from core recovery to creative arts and independent living skills. Consumers have access to on-site educational opportunities and representatives from numerous faith groups visit our facility each week, allowing for spiritual growth.

Crestwood Manor is a skilled nursing facility dedicated to helping consumers transition to a more independent setting and providing the most comfortable environment possible for our longer-term residents. All consumers have personalized recovery programs that are designed to help them either work toward a lower level of care or maintain the highest possible level of functioning, and they receive assistance to help execute the activities of daily living. To ensure consumer comfort and continuity of care, we also offer palliative care, supporting consumers as they spend their final days with us.

Crestwood Recovery and Rehabilitation Center is accredited by the Commission on Accreditation of Rehabilitation Facilities (CARF), which requires a higher standard of care than the licensing standards in California. Crestwood believes this accreditation demonstrates our unwavering commitment to achieving and maintaining the highest level of care and rehabilitation.
